Abstract : Peripheral blood eosinophilia is a well-recognized but rare manifestation of cancer. It may occur as a component of the disease (myeloproliferative disorders or chronic eosinophillic leukemia) or as part of a paraneoplastic syndrome (mainly solid cancer). We present the case of a female patient with metastatic breast cancer with eosinophilic hyperleucocytosis. The main entities that were considered as a differential diagnosis were helminth infections, atopy, idiopathic hypereosinophilic syndrome, and underlying malignancy. Absence of any cytogenetic abnormality, response to deworming, hydroxyurea, steroids, Imatinib Mesilate, and the treatment of primary malignancy favors this rare paraneoplastic manifestation of the primary tumor.
