Abstract : This case report highlights the effective management of a maxillary lateral incisor with perio-endo lesion precipitated and complicated by the presence of a deep palatoradicular groove extending up to the root apex. Despite an apparently poor prognosis, the tooth was successfully managed by a collaborative endodontic and surgical periodontal therapy. The periodontal ligament attachment and periradicular healing were appreciated both clinically and radiographically at 3-month follow-up.
