Approach to Variceal Hemorrhage

Abstract : A 40-year-old male, an alcohol consumer (120 gm/day for 5 years) now presents episode of hematemesis followed by alteration in sensorium. There are no other significant medical comorbidities. There is no history suggestive of abdominal distention, decreased urine output, shortness of breath. On examination he is hypotensive (BP 90/55 mmHg), has tachycardia (HR 126/min) deeply icteric, and has parotid enlargement with bilateral pedal edema. There is presence of asterixis. Per abdominal examination is remarkable for an enlarged firm irregular liver.
