The aim of the programme will be to provide training to freshly graduated plastic surgeons ,in the field of composite tissue allotransplantation. Training in hand , face, penile, abdominal wall, uterine and tracheal transplant surgery.
there are few centres in the country performing vascularised allotransplant, and none providing a fellowship course in it.
We are going to start the VCA program by the month of March- April and we expect with our experienced team and the number of patients waiting we expect a very high volume of cases which may reach at least one transplant a month.
A fellow will attend outpatient clinic and evaluate the patients, (amputees and other candidates presenting for transplant). They will be guided regarding a.
a. selection of suitable patient for transplant
b. counselling
c. short listing of candidates
d. pre operative work up which includes studying level of amputation, vascular studies where and when required, work up for infections and patient specific antibodies etc.
e. Charting plan for the surgery.
a. Guidance regarding immediate preoperative care, forming team (to alert the nephrologist, anaesthetist, orthopedician and paraclinical staff) , sending samples for matching , counselling, consent and immunosuppression. After 5 cases will be expected to perform such duties independently.
b. Assisting in donor preparation, harvesting, on table dissection, recipient preparation and transplant process. Once the fellow shows satisfactory skills, will be allowed to participate in a more responsible capacity in the team.

a. Guidance regarding post operative monitoring.

c. Tracking immunosupression and coordinating with the team (which includes intensivist and nephrologist)
Coordinating with physiatrist regarding post op physiotherapy mobilisation , ambulation etc.
Coordinating with nephrologist regarding immunosuppression, baseline post transplant histology, coordinating with pathologist. Training in diagnosing, evaluating and managing rejection.
